Part time Diabetes Nurse Educator or Certified Diabetes Care and Education Specialist (CDCES) position available. Lamprey Health Care is an Association of Diabetes Care and Education Specialists (ADCES) Accredited Diabetes Self-Management Education and Support/Training (DSME/T) program. This individual provides patient-centered diabetes education and support(DSME/T) for Lamprey Health Care patients. Responsibilities include review and management of diabetes population data, assessment and patient-centered one on one DSME/T to Lamprey Health Care patients, and assist in developing outreach programs for the community. Also serves as a resource for the clinical staff and provides and coordinates staff diabetes education.

Required qualifications include current New Hampshire Registered Nurse license, certification or willing to obtain certification within 3 years upon hire.
