We are looking for a full-time Community Health Worker for our Nashua location starting range $ 20.00 - $ 22.00

Responsibilities will include

• cultural outreach to assist patients in understanding their plan of care
• coordinate patient health services
• educate patients on adopting positive health choices
• assist patients with follow up medical appointments.

This role has a large focus on conducting community outreach and health education to inform the community of the many services offered by our dynamic integrated health team.

Bachelor’s Degree desired; Community Health Worker trained ideal. Bilingual preferred. Must have reliable transportation.
