Are you a positive individual who enjoys a team-oriented environment? We are looking for a person with excellent interpersonal and community engagement skills to share resources, conduct outreach and build community relationships.

The Seacoast Public Health Network (SPHN), a program of Lamprey Health Care, is seeking a Community Health Worker (CHW) who promotes resources designed to increase access to quality health care in the 23 towns that comprise our public health region.

This position is both remote and community-based. Will need to be in commuting distance to the Raymond, NH and the 23 towns that comprise our public health region. This includes in-person activities and community outreach. The CHW will be responsible for the following essential job functions:

    • Connecting community members to culturally and linguistically competent COVID-19 testing and vaccination sites
    • Cultural mediation among individuals, communities, and health and social service systems.
    • Care coordination and system navigation.
    • Coaching and social support by advocating for individuals and communities.
    • Identifying and helping to mitigate barriers in health care access such as transportation, language, and childcare.
    • Educating healthcare providers and stakeholders about community health needs.
    • Advocating for vulnerable populations or communities to receive services and resources to address health needs.
    • Building community capacity to address health issues.
    • Supporting other areas of PHN work including substance misuse prevention and emergency preparedness.

Required

  • Position requires graduating from Community Health Worker training (we can enroll you in a training program).
  • Ability to multitask, prioritize and operate successfully in a fast-paced environment.
  • Strong communication skills (both verbal and written) and attention to detail.
  • Excellent technology skills including use of Word, Excel, & PowerPoint, as well as knowledge of social media.
  • Well-developed diplomacy skills and respect for diverse perspectives.
